What's the Most Frustrating Gameplay Mechanic You've Encountered in RPGs?

Mechanics are the lifeline of every RPGm it's the maker or the breaker of the RPGs. Some mechanics like skill trees, loot systems, and combat systems are often very engaging but can also be really frustrating at that same time. A good mechanic can easily upgrade the game but a bad one takes it downhill.

So, what’s the most frustrating gameplay you have encountered in RPGs?

Hrmmm, only thing coming to mind right now is the draw system from FF 8, turning magic into items was dumb.

Another frustrating mechanism is that sometimes in RPGs there are some punishing difficulty levels which really increass, making it very frustrating.

True, nothing worse when you think you're doing well and then suddenly got a level 10 more monster fighting you.
